Payment Methods

After registering through this website, all payments must be made in Japanese yen by credit card or by bank transfer.

Credit Card

• Payments are accepted only through VISA and Master Card.
• Payment will be made in a single lump-sum.
• Please note that your credit card payment will be drawn from your credit card account in the name of ISS, INC., a company acting as the secretariat for this congress.

Bank Transfer

Registrants are responsible for all bank transfer charges.
Please input the 4-digit Registration Number when you are making the transfer.

Confirmation and Receipt

The Pre-Confirmation e-mail with the registration details will be sent to you after your online registration is complete.
The Confirmation e-mail will be sent to you after the payment has been confirmed.

∗ Withdrawal of credit card payments for the Early Registration fee will be scheduled at the end of every month until May 2012.
∗ If you do not receive a confirmation e-mail from the Secretariat office within 1 month from the above indication, please email the Secretariat (lmhi2012-reg@issjp.com).

∗ Receipt of your payment will be issued on the day of the congress addressed to the applicant's name as registered.

Cancellation and Refunds

If you decide to cancel the registration, please inform the Secretariat (lmhi2012-reg@issjp.com) in writing, via e-mail or FAX.
Refunds are based on the following; (Japan Standard Time).

• On and Before June 20, 17:00 JST: 100% of registration fee, minus 5,000 JPY service charges.
  ∗ For any refund by bank transfer, transfer charges should be borne by registrants.
• After June 21: No refund.

Please note that your cancellation will not be accepted if you requested documents for VISA acquisition.

Passport and Visa

A valid passport is required to enter Japan. Participants from certain countries may be required to obtain a visa to enter Japan.
If you are uncertain about your requirements, please consult your nearest Japanese Embassy or Consulate, or visit the website of the Ministry of Foreign Affairs of Japan.

The committee requires payment of the registration fee as a condition of visa issuance. The secretariat will send an official letter of invitation upon request, through the registration form below after the registration and payment is complete.
If you require a visa to enter Japan, please register first and then contact the Secretariat (lmhi2012-reg@issjp.com) by e-mail. The Secretariat will contact you in return with necessary applications.

The Registration Office will develop an invitation letter and a guarantee letter based on the information that you provide in the Visa Information Sheet and send them to you at the address specified on the sheet.
Please make your own application for visa acquisition at the Japanese Embassy or Consulate once you have received the letter.

∗ If you need to obtain a visa, please apply 3 months in advance of your visit.
The organizing committee and the registration office are not responsible for visa issuance.
